Essentials At A Glance

Program Type: Community-Based

To Make Referrals: Referrals are made through San Joaquin County crisis facilities.

# of Members: 30

Who We Serve: Adults, ages 18 and older, who have recently participated in San Joaquin County Behavioral Health Services’ 24-hour crisis response services, which includes the crisis unit, the crisis stabilization unit, psychiatric health facility (PHF), crisis residential facilities, and mobile crisis support team.

Hours of Operation: Monday to Friday, 8:30 a.m. to 5:00 p.m

About the Program: At the San Joaquin Peer Navigation program, our goal is to provide you with, or get you connected to, a full range of services to ensure continuity of care outside of the program. We provide linkage to community-based resources and medical and psychiatric services, and provide support and education to family and significant others.

We have a team of peer support professionals who will work with you every step of the way so you can achieve your recovery goals. We do this by collaborating with you to create an individualized recovery plan that is specific to your needs, hopes, and dreams.
