About the Program: In partnership with San Diego Behavioral Health Services and San Diego Housing Commission, Â黨ɫÇéƬ offers onsite supportive and case management services for persons residing at Kearny Vista Permanent Supportive Housing (PSH).

Service participation is voluntary and not a requirement of residing at Kearny Vista. Â黨ɫÇéƬ works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in Kearny Vista PSH community space; in individual units; in the supportive service office; or in the community, if appropriate. These services focus on housing stability and retention, including but not limited to:

  • Outreach and engagement services addressing housing stabilization and case management needs.

  • Behavioral health assessment, screening, care coordination, and referrals for residents at high risk, in need of more intensive behavioral health services to maintain housing.

  • Addressing ongoing behavioral health care coordination needs.

  • Offering site-wide community integration opportunities (BBQs, celebrations, tenant education, and other groups).
