We are pleased to announce that Connect III opened on March 1, 2021!

This program was opened in partnership with the .

The San Joaquin Connect III program serves adults who are disconnected from the mental health system, or are at high risk of returning to the hospital, or have struggled with transitions from the hospital back to the community. The goal of this program is to improve linkage with outpatient providers and get individuals engaged with post‐crisis follow‐up care and treatment plans.

Who is Served: Adults with serious mental illness who are at-risk of being readmitted to the hospital or are struggling with transitioning back into their community.

Hours of Operation: Monday to Friday, 8:30 a.m. to 5:00 p.m.
